Workshop: An effective coordinated health care - sure it can be done!?

A coordinated health and social care for persons with multiple illnesses and the elderly is one of the challenges for Swedish health care. Lack of cooperation lead not only to large costs and potentially worse quality of the health care, but to a deterioration of the experience and life quality for the patients. Forum för Health Policy is launching five good advices to attchieve a coordinated health care, which this workshop is going to adress. The workshop will have shorter presentations and a lot of time for exciting dialouge.
